Investigation Summary
Employees #1 and #2 were attempting to remove the lift cylinder from a Clark forklift. They raised the forks to the extension bars, then secured the forks with a chain. While Employees #1 and #2 were working, the chain became unsecured, causing the forks to fall approximately 3 to 4 ft and strike the employees on top of the head. One employee sustained serious injuries.
