Investigation Summary
Employee #1, who had recently been made a permanent employee of a package and shipping company, was operating a power extender w/c, a telescoping conveyor used to unload trucks. Operating the controls with his left hand, the employee protracted the extender to contact the roller conveyor inside the truck. Employee #1 slipped on a package and used his right hand to stabilize himself by resting it on the edge of the conveyor. As a result, his hand was caught between the conveyor and the front of the power extender. Employee #1's right index finger was almost completely amputated.
