Investigation Summary

Investigation Nr: 170883581
Event: 01/15/1995
Employee struck in eye by flying piece of metal

Employee #1 was in the maintenance area checking on the progress of a bearing that was being removed from a gearbox pump motor in a shaft. An employee had put the shaft in the shop vise and was using its gear as a holding surface. The shaft was placed horizontally to the bearing, which had been expanded by heat. A coworker was striking the inner bearing race with a drift punch and a hammer. Employee #1 heard a noise, turned to see what it was, and was struck in the left eye by a flying piece of metal from the bearing. Employee #1, who was not wearing safety glasses or a face shield, was hospitalized for three days for eye surgery.
