Employee injured in fall as ladder slips

Employee #1, of Rogers Plumbing, was working on a sprinkler riser flange, attempting to remove several bolts, while standing on a 6 foot stepladder. He was not able to get access to the bolts in that position and closed the stepladder and leaned it against a guardrail that was around the sprinkler riser. As he worked on the ladder in its new position, the ladder began to slip out from under him, causing him to fall. He struck the metal guardrail, breaking two ribs and collapsing his lung.
