Employee's ankle and foot crushed by forklift

At approximately 2:30 p.m. on July 12, 1995, Employee #1, of U.S. Growers Cold Storage, Inc., Los Angeles, CA, was test driving a Raymond forklift in the repairing area. His left foot slipped and became caught between the body of the forklift and an adjacent pole. He sustained a crushing injury to his left ankle and foot. He was taken to Garfield Hospital, where he was hospitalized for two days. He is now being treated as an outpatient.
