Investigation Summary
Employee #1 was removing excess strips of corrugated cardboard as it was being cut in a triple machine that cuts the strips using circular knives mounted on a shaft. A vacuum machine is normally used to pick up the strips, but it broke down that day and Employee #1 was assigned to perform the task in the interim. He was pulling on the strips from the inlet side of the machine as it was cutting when one of the strips was pulled back into the machine. He immediately tried to retrieve the strip with his right hand but his glove became caught on the knives. His thumb was completely amputated and his index and little fingers were partially amputated.
