Investigation Summary
Employee #1 was cutting an 18 in. by 6 in. by 3/4 in. piece of plywood with a 10 in. powered wood table saw. The blade was not adjusted to the wood thickness, and a blade guard was not on the machine. Employee #1 was not using a push stick to push the wood while he was cutting. His right thumb tip was amputated when it came in contact with the rotating blade. According to the supervisor of the wood shop, the wood was too small for a guard to be used.
