Employee's fingers amputated when caught by core machine

At approximately 7:30 a.m. on May 8, 1994, Employee #1, of Universal Cast Iron Co., was working on Core Box #4, a Shell Core Blower (serial #3657-CR) manufactured by the National Acme Co. He was trying either to pick up a product as it unexpectedly fell out of the core box or to adjust or service the machine without de-energizing it. His left middle and ring fingers were caught between the two closing halves of the box and amputated. A general violation of T8CCR 3314(a) was cited.
