Employee falls as manlift dislodges from forklift

Three employees of Montebello Container Corp., a forklift driver and 2 laborers/flexo stackers, were covering double-stacked rolls of paper (each 6 feet tall). A coworker had been raised 12 feet and left on top of a row of paper rolls. The forklift driver then moved the vehicle to another row of paper rolls and proceeded to raise Employee #1 to the top of the double-stacked paper rolls. The manlift/platform used by the company did not have manufacturing specifications, a load capacity, or a serial number. The manlift/platform was picked up and then raised by the forklift while unsecured to either the forks or the mast of the forklift. The manlift became dislodged from the forks and Employee #1 fell, sustaining a fracture and head trauma. He was hospitalized for a 30 hour period upon his initial visit. Employee #1 had been employed since February 22, 1993.
