EMPLOYEE'S FOOT IS PINNED BY OVERTURNED FORKLIFT

AT 2:15 P.M. ON NOVEMBER 9, 1993, EMPLOYEE #1 WAS LOADING AND UNLOADING SEMI-TRAILERS AT A LOADING DOCK. WHILE NEGOTIATING A U-TURN AT A HIGH RATE OF SPEED, HE TIPPED HIS FORKLIFT OVER AND IT FELL ON ITS RIGHT SIDE, PINNING HIS LEFT FOOT UNDER IT. HE RECEIVED A CONTUSION ALONG THE BOTTOM OF HIS FOOT. HE WAS TAKEN TO QUEEN OF THE VALLEY HOSPITAL, TREATED, AND RELEASED.
