Employee injured in fall from footstool

On October 22, 1992, Employee #1 was sitting on a footstool next to a de-palletizer machine. When she attempted to stand on the footstool to get a better look at cases of empty beer bottles because the machine appeared to be malfunctioning, she slipped on the stool and fell. Employee #1 injured her left shoulder, left knee, and left hand. She was treated at the scene, then transported to Queen of the Valley Hospital by ambulance.
