Investigation Summary
On July 1, 1993, Employee #1 was unloading a trash can from a pallet jack. The trash can slipped and mashed the top of his right thumb against the lip of the trash can and the pallet jack handle. This injury resulted in the amputation of the distal tip of his right thumb. He was transported to Don Julian Industrial Medical Center, treated, and released.
