Employee's fingertips amputated by cutting machine blade

At about 10:30 a.m. on June 21, 1993, Employee #1, of J.T. Posey Company, was in the cutting department doing repair work on an Ace cutting machine. He was helping a coworker replace the machine's blade, when the cutting machine suddenly activated. His left hand was near the blade and the tips of his first and middle left fingers were struck and amputated by the moving blade. Employee #1 was taken to Methodist Hospital in Arcadia, CA, for medical treatment. He was released the same day. According to the employer, power to the machine had been turned off through the ON/OFF switch on the machine, but the power supply cord was not unplugged from the wall outlet. (This machine works on 120 volts AC.) Employee #1 could not be interviewed about the accident. A witness provided the information.
