Employee's finger partially amputated by molding machine

At approximately 1:30 p.m. on February 8, 1995, Employee #1, of International Medication System, El Monte, CA, was working on injection molding machine #A-5 in the molding area of building #12. He was sliding the injection unit with his right hand and pinched his index finger between the injection unit and a metallic hydraulic fitting on the machine. Employee #1 was taken to a Foothill Clinic for treatment of a partially amputated finger. During the investigation, the employer was advised to relocate the hydraulic fitting on the machine to eliminate the pinch point area. Employee #1 could not be reached for an interview. The manager reported that Employee #1 was trying to hurry while moving the injection unit forward.
