Employee injured in fall from forks of forklift

On June 14, 1999, Employee #1 was raised on the forks of a forklift to retrieve groceries from a metal rack in the stock/receiving area. The forklift moved and he fell over 10 ft to the floor, fracturing his right femur and injuring his knee cap. The employer was cited for several accident-related violations, including T8CCR 3664(a )(1), for providing no forklift training; T8CCR 3657(a), for not providing a work platform on the forklift; and T8CCR 3664(a)(4), for allowing an employee to ride on the forks.
