Employee injured when struck by chain hoisting sling

Employee #1, a regular janitorial employee of W.L. Chapman, was struck in the head by a chain sling used with a 10-ton overhead hoist being used to remove a die block from a metal forming machine. The chain sling parted at the lifting ring when the hoist controls accidentally energized the hoist in the up direction while the die block was still held by the machine. Suffering a concussion, Employee #1 required hospitalization.
