Investigation Summary
At 12:15 p.m. on August 21, 1992, Employee #1 was attempting to clean his molding press machine without shutting it off or blocking it. When he reached inside the machine to get a piece of debris, the press rose and pinned his head and one shoulder against the top of the machine. Employee #1 sustained massive injuries and died.
