Employee injures foot as forklift forks suddenly fall

At about 2:10 p.m. on January 25, 1993, Employee #1, a packer, was inside the warehouse at Group E, Ltd., of Arcadia, CA. A forklift operator had raised the forks 4 ft when suddenly the hydraulic lines jammed, making the forks unmovable. Employee #1 released the forks by hand while the forklift operator was in his seat. The forks fell suddenly and struck his left foot, fracturing a bone. Employee #1 was taken to Methodist Hospital in Arcadia for treatment.
