Investigation Summary

Investigation Nr: 170178669
Event: 12/01/1992
Employee injured in fall from electric cart

Employee #1 and two other USC employees were returning from lunch in a two-seater electric cart with a tool box mounted behind the cab at the rear of the truck. Employee #1 was sitting on the tool box, the top of which was slightly above the side walls of the truck. When the cart went around a corner at a slow speed the employee lost his balance and fell, striking his head on the concrete driveway. He was taken to the hospital and released. For the next two days, Employee #1 underwent various medical tests as an outpatient; he returned to work the following day. No seat belts or hand holds were provided on or near the tool box. Two general and two serious violations were issued.
