Three employees injured, one killed, as bent collapses

Employees #1, #2, #3, and #4 were employed by Kasler Corporation as carpenters under the supervision of a foreman. Their job assignment was to jack-up the falsework to release/remove wooden wedges on the upper platform of bent B, and then lower the falsework. The jacks set on the cap beam started to shift, and work was stopped to check the jacks. When the lower tower started to roll, the foreman and Employee #4 jumped off onto the next tower. The top cap from the lower tower on bent #7 fell off to the inside of the legs, taking all the cross braces off the lower tier and causing the whole bent to collapse. Employees #1, #2, and #3, along with their materials, fell approximately 25 to 30 feet to ground level. Debris was removed by coworkers from around the three employees, who were taken by paramedics to local hospitals. Employee #1 died and Employees #2 and #3 were hospitalized.
