Employee fractures jaw when struck by falling heater tubes

At approximately 12:35 p.m. on January 12, 1994, Employee #1, of Fluor Daniel, Inc., was cutting brackets loose from overhead heater tubes in the north end of plant 8 at Chevron's Isomea Complex, El Segundo, CA. After he cut a bracket, two heater tubes fell, striking him on the left lower jaw and knocking him off the stepladder. Employee #1 sustained a fractured left lower jaw and was taken to Centinella Hospital, Inglewood, CA, for hospitalization and treatment. The causal factor of the accident was that the heater tubes were not secured from falling while being cut loose.
