Investigation Summary
Employee #1, of National Labor Services, Inc., was operating a Temafa feeder hopper machine, which feeds shredded fabrics along a conveyor belt and into a drum. The machine jammed and stopped operating. The employee removed an inspection plate (four bolts) and reached in to clear the machine. When the materials were freed, the machine restarted, catching Employee #1's arm in the chain/sprocket assembly that powered the drum and conveyor. He was hospitalized with severe deep lacerations of the axilla (upper right arm) into the delto pectoral region and an open humerus fracture (upper right arm).
