Employee #1, of United Stationers, City Of Industry, CA, was placing boxes on a moving conveyor in aisle 10-08-2201. His right hand accidentally became caught under the conveyor belt and went into the rollers. Employee #1's right middle finger was amputated, and he sustained a laceration on his right hand.
