Employee's thumb amputated by raintape machine

At approximately 9:30 p.m. on July 6, 1995, Employee #1 was working on raintape machine #2. He attempted to change the weld belt on the machine while the power was on and the machine was running. Employee #1's right thumb became caught between the belt and roller (tip) and was amputated. After the investigation, the employee was cited for a serious violation of T8CCR 3314 (a).
