Employee's forearm fractured when caught in belt conveyor

At approximately 6:00 p.m. on July 13, 1995, Employee #1 was repairing a BC-16 belt conveyor. He turned off the machine to find the problem, but did not lock the switch in the OFF position. As the employee turned the roller by hand, he accidentally hit the power switch to the ON position with his knee. Employee #1's left arm was caught in the belt and roller, resulting in multiple fractures to his left forearm. After the investigation, the employer was cited for the serious violations of T8CCR 3314(f) and T8CCR 3328(g). Other violations were found and the employer was cited accordingly.
