Investigation Summary
Employee #1 was teaching a coworker how to operate the levels and run the fork lift truck. The coworker was driving the fork lift truck while Employee #1 was standing on the forks of the fork lift that were approximately 10 in. above the floor, giving instruction on how to proceed in driving safely. At an intersection, Employee #1 told the coworker to sound the horn twice and the coworker did comply. At that point Employee #1 and the coworker saw another fork lift truck approaching directly towards them. Employee #1 immediately jumped out of the way, but the corner of the wooden pallet from the other fork lift truck struck his left foot, causing an open fracture. Employee #1 was hospitalized.
