Investigation Summary
The original narrative covers two separate incidents. Incident One: On October 24, 2000, Employee #1, a machine operator, was cleaning the cylinder plate roller with a rag while the machine was running. Employee #1 was not provided with an extension tool for cleaning the running rollers of the machine; the establishment did not have a lockout procedure and the machine was not guarded to prevent an employee's hand or fingers being caught between the rollers or between the guard and the rollers. The in-running roller of the machine caught Employee #1's right hand. Employee #1 suffered an acute burn second-degree burn and was hospitalized. Incident Two: At approximately 8:00 p.m. on November 18, 1999, Employee #1, a machine operator, had just finish adjusting the impression-cylinder-roller of machine Number 24 and had run several parts when he inadvertently placed his right hand on the plate-cylinder-roller. The machine did not have a guard for the in-running rollers. These two cylinders rollers are in-running and they caught and crushed Employee #1's right hand. Employee #1 suffered the amputation of the tip of his right ring finger and a crushing injury to his middle right finger. Employee #1 was treated for his injuries but was not hospitalized.
