Investigation Summary
On July 27, 2005, Employee #1 was checking to see if the pulley was loose in the mixer. At the time he checked the belt of the pulley, it wasn't moving. The pulley intermittently starts and stops when the mixer changes direction. Employee #1 indicated the belt of the pulley was not moving and he forgot that the machine was still on. The belt and pulley drive which were located less than 7 ft, were partially guarded and accessible. According to Employee #1, he has fixed belts of the pulley drives in the past and was told to turn it off prior to changing it or touching the belt. He was hospitalized.
