Employee fractures leg, pelvis after fall from work platform

At approximately 3:00 p.m. on August 3, 2000, Employee #1 and his supervisor were engaged in dismantling a storage rack inside the THARCO warehouse. The employee fractured his left leg and pelvis after he fell from the elevated forks of a Toyota forklift. Employee #1 was standing on a wooden pallet upon the forks of a TOYOTA forklift, Serial Number 24992, and his supervisor was operating the forklift. According to Employee #1, he did not have fall protection when he fell about 8 feet from the elevated forks of lift truck to the cement floor. A standard platform with guardrails was not provided for him to stand and work while in the elevated position. The employee was hospitalized with his injuries.
