Employee's hand is broken and burnt when caught in machine

At approximately 3:00 p.m. on April 24, 2000, Employee #1 was operating a Battenfeld Gloucester 418 bag separator machine Number 6, (Model Number 25435-G1, Serial Number 31760-03). The plastic sheet had broken and Employee #1 was attempting to rethread the plastic into the inlet side of the bag separator. She turned the handle to open the nip rolls but the lever did not open the rolls. She then walked to the inlet side of the machine and opened the fixed barrier guard that was guarding the in-running rolls of the bag separator. This fixed barrier guard was equipped with an interlock so when the guard was lifted the machine would stop. However someone had by passed the safety circuit by taping the interlock switch down. She then attempted to push the plastic sheet through when her right hand became caught in the rollers. She was hospitalized with a broken and burnt right hand.
