Employee's Fingers Are Amputated When Caught in Conveyor

At approximately 1:00 a.m. on November 15, 2000, Employee #1 was in the process of stripping 1 1/2-inch SS 280 hose on the stripping table #1. Employee #1 rolled the mandrel with finished hose onto the stripping table #1 and cut both ends of rubber hose to get it freed from the mandrel using a standard rubber knife (1" wide x 6" long blade). He clamped down the mandrel on one end with No. 27 Rigid clamp and tied a nylon strap around the hose. He then made an incision on the rubber hose with the hand knife to apply air nozzle and used the compressed air to separate the hose from mandrel. Employee #1 turned on the stripping table #1 and the conveyor belt of stripping table removed the rubber hose from mandrel. The hose got stuck, so Employee # 1 removed the air nozzle from the rubber hose and turned off the stripping table #1. He adjusted nylon straps around the hose/mandrel and turned on the machine. He re- inserted the air nozzle to the hole on the rubber hose and placed his left hand at the end of rubber hose to ensure the air was not leaking out. His left hand got tangled with the nylon strap and it was pushed against the edge of rubber hose as the conveyor of stripping table moved. Employee #1 screamed for help and another employee came over to stop the stripping table. Employee #1 was taken a medical center and he was released the next morning after getting surgery on his left hand. His left index and middle fingertips were amputated due to the accident.
