Employee injured when adjusting lift gate

On October 17, 2002, Employee #1, a mechanic/welder with Van's Equipment and Repair, was adjusting the speed of a lift gate attached to the back of a van type truck. The lifting mechanism activated, possibly due to work performed on its electrical wiring, and crushed him. Employee #1 was hospitalized at County USC Medical Center for a pelvis injury. The accident was caused by failure to lock out the power source or block out the mechanical components prior to repair work.
