Investigation Summary
On March 2, 2001, an was operating the Kogyo Co. Limited Model a 16 dumpling machine. He noticed that the dough was balling up so he stopped the machine using the emergency stop, and cleared the balled up dough out of the way. The employee then restarted the machine to attempt to refeed the dough. The first time he tried to reefed, the dough it didn't take so he cut the dough and tried again. This time, he used his hands to guide the dough into the bottom of the machine where the cutter is located. There was no guard to prevent the operator from reaching into the point of operation. The employee got his right middle finger cut by the Kogyo Co. Limited Model a16 dumpling machine cutter. He was transported to the mercy medical center where he was treated for amputation of the right middle finger.
