Employee Loses Tip Of Finger - Nose Bar Pinched His Fingers

At approximately 4:50 p.m. on March 23, 2001, Employee #1 lost the tip of his left index finger when the nose bar slipped out of his hand and pinched his finger between the bar and the kreuter enrover. Three witnesses observed the incident. The machine involved in the incident was the Line Number 5 kreuter enrover and the nose bar for the second tunnel conveyor. The nose bar and conveyor were supported by two pipes, one on each side near the enrover. A hydraulic jack or sometimes shims were used to compensate for the height difference between the nose bar, conveyor and the kreuter enrover. This machine had been in operation for about three months when the incident occurred. At the time of the incident, Employee #1 and his coworker were aligning the Line Number 5 nose bar of the Number 2 tunnel conveyor to the kreuter enrover. His coworker was aligning the pipe on one side of the machine using a pipe and a hydraulic jack. Employee #1 aligned the pipe on the other side of the machine. Employee #1 placed the bottom of the pipe on top of some washers and shims, and the top of the pipe was placed under the conveyor. Employee #1 attempted to hold the pipe in place with his right hand and lower the nose bar to the enrover with his left hand. The nose bar slipped out his hand and pinched his left index finger between the nose bar and the end of the kreuter enrover. Coworkers applied pressure to the wound and escorted him to the office. Another coworker then drove Employee #1 to the local medical center. Employee #1 was transported to the Irwindale industrial clinic and later transferred to the West Coast Orthopedic Medical Group were he was treated for amputation of the tip of his left index finger.
