Employee injured in fall from pallet

At about 8:10 a.m. on November 28, 2001, Employee #1 of Bmk Inc, located in Commerce, California, was operating a crown order picker Number 25. Employee #1 was standing on a standard wooden pallet about 6 ft above the cement floor of the warehouse. He was removing small boxes from the pallet and placing them on shelves. As Employee #1 moved toward the front of the pallet, it tipped down a little causing him to lose his balance and fall off the pallet. Employee #1 landed on his head. Employee #1 was transported to the USC Medical Center and later he was transferred to Downey Community Hospital. Employee #1 spent five days in the hospital due to a fractured right wrist, and a fractured skull in two places. At the time of the accident Employee #1 was wearing an approved safety harness and an approved retractable lanyard was attached to the frame of the order picker. The employer enforced the use of fall protection. Employee #1 said that he always used fall protection but must have forgotten to attach the lanyard to his safety harness that time. Another employee said that most of the boxes had been removed from the elevated pallet prior to Employee #1's falling.
