Investigation Summary
At approximately 8:25 a.m. on October 10, 1993, Employee #1, while preparing cakes, was working with an air-activated cake depositor which operates at 70 to 100 psi. He placed his finger between the two actuators and the metal adjustment bar. When the machine cycled, his right ring finger was crushed below the 1st joint.
