Event: 01/21/1993
Employee lacerates head in fall from dock

At approximately 9:30 p.m. on January 21, 1993, Employee #1 had stopped to transfer freight at dock #10, an open dock. The dock surface was smooth and covered with water from the heavy rain and wind. The concrete dock lip did not have an angle iron cap and, in fact, had been chipped which gave it a round tapered edge. As Employee #1 was looking for steps to descend, he slipped off the edge of the dock and fell 4 1/2 ft, striking and lacerating the back of his head on the concrete vertical wall. He was kept at the hospital primarily for observation and was discharged after 2 1/2 days.
