Investigation Summary
Employee #1 was attempting to repair a fruit feeder machine. The equipment was electrically locked out and disengaged, but Employee #1 forgot to disengage the air (the equipment is an air-over-electric system). While Employee #1 was diagnosing the problem, the machine cycled, causing a linkage rod to strike his finger. The finger was not amputated, but the doctor did trim the bone.
