Event: 03/02/1993
Employee's finger caught in carrot chopper

Employee #1, of J. R. Wood, Inc., was feeding carrots into a carrot chopper. Because the carrots were hung up, the employee attempted to remove the jammed product with her hand with the chopper energized and cycling. When the product was removed, the machine cycled and her right ring finger was caught/struck by the cutting blade guard, which is automatic and pneumatically controlled. Employees are provided with verbal instructions concerning clearing jams and a piece of PVC pipe is located on the line to be used by an employee when clearing a jammed chopper. Employee #1 confirmed that she had received verbal instruction to turn the machine off when clearing jams.
