Investigation Nr: 170608822
Event: 03/12/1993
Employee dies after being caught in tractor fire

At approximately 9:00 a.m. on March 12, 1993, Employee #1, a foreman/tractor operator, attempted to pull a heavily-laden highway trailer across soft ground, using a jury-rigged chain wrapped around the trailer axle and attached to the tractor. The high hitch point caused the tractor to upend and pin Employee #1. Fuel that had spilled from the tractor caught fire and severely burned Employee #1, who died the following day.
