Investigation Summary
Employee #1, of BMC West, was operating a radial arm saw. He had the guard adjusted above the worked piece at a height outside the manufacturer's recommendation of 1/4 in. or the height of the fence, whichever is higher. As Employee #1 was manufacturing a two-angled web piece with the guard out of adjustment, his left hand was pulled into the saw and his finger was amputated. Employee #1 required hospitalization.
