Employee struck by crosshead assembly of forklift mast

Employee #1 was repacking the seals on a single-stage lift cylinder. To block up the mast crosshead, he cut two pieces of channel iron measuring approximately 2 in. by 15 in. (the actual size of the material was 2-in. by 1.69-in. by 15.75-in. with a 0.125-in.-thick wall). One of the pieces (he did not recall which) did not fit snugly and was loose. Employee #1 installed one piece in the left crosshead guide and the other in the right crosshead guide. He also used a 3/8 in. welded-link steel chain, which he wrapped around the mast and the mast crosshead assembly. He secured the chain with a 4.5-in.-long by 0.375-in. bolt, two washers, and a nut. He started the forklift motor to force the packing up, and then shut off the engine and began cleaning out the old packing with his right hand. It was during the cleaning that one of the pieces sprang out, and the mast crosshead fell. Employee #1 suffered fractures to his hand for which he was hospitalized.
