Employee's finger crushed in valve

On July 1, 1994, Employee #1, a maintenance supervisor, returned to a valve inspection/repair that he was supervising. In his absence, the work crew under his supervision had reassembled the valve actuator. When he reached into a partially-dismantled butterfly valve casing to inspect the valve seat seal, the air supply valve was actuated by a member of the work crew, causing a serious crushing injury to his right index finger and surgical revision to the middle knuckle. The employer is being cited for a serious accident related violation of T8CCR 3314(b).
