Employee injured in fall through false ceiling

Employee #1, of J. R. Woods, Inc., stated that his supervisor had given him permission to change a valve after he finished helping a coworker. Employee #1 said that the supervisor told him to trace the line out and turn the water off at its source, but did not specify the exact location of the pipe nor offer to take him to the main cut off valve. Employee #1 ascended the steps to the catwalk, stepped onto the catwalk and turned right, and walked to the first intersection and then turned left. He walked a short distance and noticed where the pipe came through the ceiling. Since he did not observe any valves on the wall where the line came through, he stepped off the catwalk and walked on a beam until he reached the area where the line came through. He still did not see a valve. He stepped on the plywood ceiling to determine if it would support his weight. It appeared that it would, but the ceiling broke, causing him to fall to the ground. Employee #1 required hospitalization for a dislocated hip. Employee #1's recollection of the accident did not match his supervisor's written statement.
