Employee's fingertip amputated by shaper blade

On September 17, 1996, Employee #1, a furniture maker, removed the guard from a 4 in. diameter shaper blade to adjust the cutting head. He then ran through a 6 in. wide by 3/4 in. thick by 10 in. long test piece of pine without replacing the guard. The tip of the little finger on his left hand was amputated by the rotating blade.
