Employee's arm injured when caught in conveyor pulley

On July 24, 1996, Employee #1, an experienced clean-up worker, reached into the nip point of a conveyor pulley to clean while the machine was running. He suffered serious lacerations and a fractured ulna in his right arm. Employee #1 violated a specific lockout/blockout procedures for the conveyor. A lockout padlock and disconnect switch was mounted on the wall next to the conveyor, and a written procedure for cleaning processing equipment that required lockout/tagout was in place. Employee #1's initial and subsequent lockout training was documented.
