Employee injured when struck by unsecured grab hook

On August 15, 1995, Employee #1, a maintenance mechanic for Newark Sierra Paperboard Corp., was using a grab hook with a winch and come-along to pull a chain conveyor into position in a pit under a paper machine. The 10 lb grab hook broke loose and snapped back by the attached, stretched out, 3/4 in. nylon rope, striking Employee #1. He sustained a ruptured spleen and broken left ribs and was hospitalized. The hook had not been secured as required to minimize the hazards. The accident was caused by breakage and a release of mechanical energy.
