Investigation Summary
At 2:00 a.m. on May 18, 1995, Employee #1 was cleaning at a poultry processing facility. He was attempting to pull turkey feathers out of a quill puller machine and received a partial amputation of the right thumb and tip of the right index finger. It was not determined whether the machine was on at the time of the accident or if someone inadvertently turned it on. The machine is comprised of two in-running 3 in. diameter, 2 ft long rolls with a 1 1/4 in. slotted opening 51 in. above the floor. The employer has an effective safety program and energy control program.
