Event: 11/10/1995

Employee fractures hip in fall from stacked boxes

At approximately 9:30 a.m. on November 10, 1995, Employee #1 and coworkers were covering an approximately 24 ft by 60 ft block of corrugated boxed units, six units wide and fifteen units long, with black plastic for weather protection. Employee #1 had moved from the top of the block to a part where the units were stacked two-high and applied plastic covering as required. The lift truck that provided access down was at the opposite end of the block so Employee #1 proceeded to lower himself the approximately 8 ft to the ground. He had lowered himself to the point where his chest was about even with the top of the stack when the wind caught the black plastic. As Employee #1 dropped down, he slipped on the blowing plastic and landed on his side. He was hospitalized with a fractured hip. .
