TRUCK DRIVER RUN OVER BY A FRONT-END LOADER

On January 29, 1998, Employee #1, a truck driver, was struck by a front-end loader operated by an employee of Ampine. The loader operator had a load of material and was driving to the storage area to transport the load to the rejection pile. The loader operator indicated he had good visibility, even though it was a dark rainy, windy morning. The accident site was a down-loading raw material storage area for trucks that deliver wood chips and sawdust. After the load is dumped, the driver retrieves the tarp from the bed of the live bottom trailer. As the loader operator entered the south side of the storage area, he looked in the direction where the truck and trailer had parked. The operator felt that he had a clear view of his path and did not see the driver. He began to drive on the black-topped area. He suddenly felt a bump on the slab near the storage area. The loader operator stopped the vehicle and got out. At that moment, he realized that an accident occurred. The driver was fatally injured in the roadway as a result of the impact.
